I would like to say a huge thank you to all the Shine workers for their amazing efforts in supporting AA transitioning out of hospital.
AA has had a very lengthy admission at Chase Farm and his discharge planning required a lot of hands-on support to ensure that it was successful in helping him towards maintaining his mental wellbeing as he moves forward with his life. AA himself has reported positively each week from the support he has received from the staff at Carlton Road. I am sure you will continue the great working relationship you have built with us, the inpatient team and with our community colleagues.
